Sammael IS still alive, RJ started killing to many Forsaken before the series was so popular and he is no bringing them back for more books and the Final Battle. This is why both aran'gar and Osan'gar came back, Cyndane is lanfear, and Ishy is Moridin, and what not. Rahvin is gone, balefired period! so is Be'lal. but Sammael lives on, as a character we all know and suspect, but didn't ponder.
Sammael is Mazrim Taim.
Both are blonde, Sammael could disguise his face, and power, and not go mad. Would also explain why Rand hates him and LTT do. We know Taim is mis-using the Asha'man and training a select few to be Bad. Alsio everyone suspected Demandred to be Taim but we now know Dashiva was. So who is Taim? easy, Sammael.

I also think Sammael is not dead. He wove a duplicate of himself (Hey, who knows what the Forsaken can and can't do!) so when when Rand killed this image Rand would think Sammael was dead and would leave him alone and also not disobey the DO orders. Samma
